Notable Curriculum Form Format Rules
Many items can be checked by the program and/or GFO Coordinator before
they get to the GFO Curriculum committee. Failure to complete the
forms properly delays the application approvals.
- Course changes with new titles and/or numbers
- The new title/number is put on the top of the form;
- Bothell is sufficient for "College or School"
- Helpful form filling hints
- You can find a checked box under Insert menu.
- Choose Symbol in the font menu.
- Choose Wingdings (fourth from the bottom).
- The checked box is in the last row, 2nd from the right end.
- Justification
- These must be strong statements.
- Do not rely on the members of the
curriculum committee knowing what you mean.
- Do not rely on your program colleagues on the committee explaining
what you mean.
- The justification must be clear to the Seattle campus as well.
- Please emphasize the enhanced learning goals and outcomes
for the class.
- Knowledge Areas and QSR
- Don't forget to check if the course will satisfy either
the VLPA, I&S, NW, or QSR requirement.
- Course Description
- Must be double-spaced
- No more than 50 words (every word is counted including prerequisites)
- Abbreviated title for transcripts
- It is used for all published course schedules, so be clear!
- It goes after the colon (for new course application), not on the line
- No more than 19 characters (including spaces)
- Credits (on top of back side)
- Do they add up?
- Undergraduate:   typically a 5-credit lecture has 5 total
contact hours, 10 outside hours (2 times contact hours),
for a total of 15.
- Graduate:   typically a 3-credit lecture has 3 total
contact hours, 9 outside hours (3 times contact hours),
for a total of 12.
- A syllabus is REQUIRED with:
- New course application

- New permanent course

- Major change in course content

- Conversion of temporary course to permanent

- Temporary course for more than a year
- Course change application

- Prefix/Number -- when level is increased

- Credit -- when credit is increased
