Independent contractor Instructions

The steps you need to take to get paid as an independent Contractor are as follows.

1) Create a SAM individual user account and then register yourself as an individual entity at www.sam.gov .  Once you have completed the registration process you will be provided with a DUNS number.

2.) Use the DUNS number to fill out the attached VA Vendor Add Request and return it to Wesley via email.  Only fill in the yellow sections. Do not include you SSN.  This form will allow us to add you as a contractor to the VA database of approved contractors.

3) Fill out the attached SF3881 form and return it to Wesley via email.  Providing your bank information will allow the VA to pay you. Please don’t add your social security number or bank info to the SF3881 form.  For security reasons we will get those from you over the phone.

4. Use this online system to make an appointment to get a VA ID badge (PIV card).  See PDF instructions, “VA Fingerprinting  Badging On-Line Scheduling Tool Guide.pdf”. See how to get a VA PIV badge for more information.

5. Jan will be contacting you to send you the without compensation (WOC volunteer) paperwork and the safety training instructions.
