- Aortic stenosis
- Severe CHF
- Increased preload
- Endogenous catecholamines
- Hyper-inotropic state LV dilation
- Decreased myocardial O2 delivery
- Hypoxemia
- Hyperviscosity, e.g. polycythemia
- Anemia, inc. methemoglobinemia
