605 pts with h/o one MI randomized to Mediterranean diet (< 35% of calories from fat; < 10% of calories from saturated fat; < 4% from linoleic acid; > 0.6% from alpha-linolenic acid) vs. "usual care." At f/u (avg. 2y), sig. lower rate of CV death or nonfatal MI (1.32 vs. 5.55 per 100 pt-yrs). RR 0.24 for all endpoints (inc. periprocedural infarction, nonfatal CHF, unstable angina, CVA, PE).
