LOCATION
The UW Dermatologic Surgery and Dermatology Center
is located about 10 blocks west of University of Washington
Medical Center (UWMC) main hospital. Easy access from I-5
and abundant covered parking are among the conveniences of
our private, pleasant location. The address beginning mid
March 2003 is 4225 Roosevelt Way NE, Seattle, Washington
98105. Please note that there are two adjacent UW medical
buildings on Roosevelt (4245 and 4225 Roosevelt Way). The
Dermatology Center is in the more Southern of the two
buildings (4225 Roosevelt Way).
Directions and maps can be sent to you (also see
map below). Please note that the UW Cosmetic
Surgery Center where Dr Berg also works is located
conveniently in the building next door to the dermatology
center . (4245 Roosevelt Way)

Parking at UWMC
Roosevelt
Validation for reduced rate parking is provided for
UWMC-Roosevelt patients and visitors for the convenient
underground parking garage (one in each of the two
buildings), accessed from Roosevelt Way N.E. Please remember
as you come that UW Medical Center - Roosevelt is housed in
two adjacent buildings (4245 and 4225 Roosevelt Way). The
Dermatology Center is in the more Southern of the two
buildings (4225 Roosevelt Way). The garage accommodates
vehicles up to 6-feet, 5-inches tall.
The maximum cost for daily parking with validation is
$4.00. Daily Parking without validation is $7.00
If your vehicle is taller than 6'5", please park in the
garage (located behind the building on 9th Ave NE), which
has spaces for vans modified for wheelchair users. To enter
this garage, drive past UWMC - Roosevelt (one way, going
south), turn right onto NE 42nd St. Turn right onto 9th Ave
NE. Our garage is about halfway down the block; the second
garage on the right and is marked "UWMC-Roosevelt".

If
You're Taking The Bus
Many bus routes stop right outside, or near
UWMC-Roosevelt: Southbound routes (stop right outside
UWMC-Roosevelt) 67, 74, 302, 305, 355 Northbound routes
(stop one block east on 11th Avenue N.E.) 67, 74, 302, 305.
For specific information on how to reach UWMC-Roosevelt from
your location, call Metro's 24-hour Rider Information at
206-553-3000. Community Transit buses (Snohomish County)
also stop nearby - call 206- 353-RIDE for information.
Getting
To and From the UW Medical Center Main
Hospital
There is a Free shuttle bus with frequent runs to
UWMC-Roosevelt. At UWMC Main Hospital, the shuttle bus stop
is located on Level 1 at the back of the medical center,
near the back entrance to the Plaza Cafe. The shuttle stop
at UWMC Roosevelt is outside the front entrance of the
4245 Roosevelt Way NE Building. Bike racks are
available at UWMC-Roosevelt. It's just a 20-minute walk from
UWMC (Main Hospital) to UWMC-Roosevelt.

A CONSULT
If you would like more information about the University
of Washington Dermatologic Surgery Center, or to book a

can also be referred to our Center by your family physician,
dermatologist, or other medical specialist.
INSURANCE
AND FEES
Treatment of skin cancer and other diseases of the
skin are usually covered by most insurance plans. In most
cases, cosmetic procedures are not covered by insurance.
During your consult, any fees related to cosmetic or
uninsured services will be discussed with you.
HOTEL
ACCOMODATIONS
If required, there are hotel accommodations nearby, some
across the street. Feel free to ask us for a list of
preferred hotels. Discounts are available for UW Patients
needing to stay overnight because of surgery.

INFORMATION (Cash Machines, Food, Lab
Services)
There is a cash machine located in the 4245 Roosevelt
building located just north of the Dermatology and
Dermatologic Surgery Center. An espresso stand located on
the first floor of the 4245 building serves a variety of
coffees as well as a selection of soup and sandwiches. There
are vending machines behind the information desk of the 4245
building on the first floor. There are other eating
establishments within easy walking distance of the
Dermatology Center with a variety of cuisines available.
Laboratory Services are available from 8am to 5:30pm
Monday to Friday on the first floor of the 4245
building.
There are no retail pharmacy services available at UWMC
Roosevelt. You may fill your prescriptions at our main UWMC
site on 1959 Pacific St or at your neighborhood pharmacy of
your choice.

Traveling
South on I-5
Take the N.E. 45th/50th Street exit (#169). Stay to the
left so you will exit I-5 at N.E. 45th Street.
Make a left turn at N.E. 45th Street. You'll be traveling
east toward the University of Washington.
Turn right onto Roosevelt Way N.E.

Take the I-5 North exit from westbound 520 and stay in
the right-hand lane.
From I-5 North, take the N.E. 45th Street exit (#169). It
will be the first exit you reach once you get on I-5.
Stay to the right as you exit until you reach N.E. 45th

Make a right turn at N.E. 45th Street. You'll be
traveling east toward the University of Washington.

Travel west on N.E. Pacific Street. Stay in the
right-hand lane.
Make a right turn at 15th Avenue N.E.
Travel north on 15th Avenue N.E. in the left-hand lane
until you reach N.E. 45th Street. There is a left-hand turn
lane and a traffic signal there.
Turn left at N.E. 45th Street.
Travel west for five blocks.
Turn left on Roosevelt Way N.E. There is a left-turn lane
and a traffic signal there.

Once
You're on Roosevelt Way NE
Travel south on Roosevelt Way N.E. for two blocks.
UWMC-Roosevelt (4245 and 4225) are the two large red
brick buildings on the right-hand side, just after you see
the street sign for N.E. 43rd Street. Remember the
Dermatology Center is in the southern of the two buildings
(4225)
As soon as you see the building, get ready to turn right
into our underground parking garage.
