Disability Accommodations
To request academic accommodations due to a disability, please review the resources and procedures available at
Disability Resources for Students, 011 Mary Gates Hall, (206)543-8924 (V) or
(206)543-8925 (TTY). To use these services, you must first fill out an application and set up your online myDRS account. Once this is done, you need
to
fill out the online Testing Contract form which will permit you to take all exams for this course in the DRS Testing Center in 011 Mary Gates Hall.
I can help
you fill out this form or you may do it online. In either case, I will review it before it is accepted by DRS. Note that you may take the exams any
time of the day on which we have a scheduled exam. These dates are shown on our class web site. Five days prior to each scheduled
exam, you are responsible
for scheduling the time to take the exam using your online
myDRS account. And, for every exam taken at the DRS testing center, you are required to agree that the timing is consistent with the exam dates
previously approved on your Testing Contract. Follwoing these procedures will enable you to take your exams at the DRS Testing Center on the
regularly scheduled exam dates.

Plagiarism, cheating, and other misconduct are serious violations of your
contract as a student. We expect that you will know and follow the University's
policies on cheating and plagiarism . Any suspected cases of academic misconduct
will be handled according to University regulations. More information, including
definitions and examples of Academic Misconduct, is available at this link.
