AM1Hematology II. Anemia Due to Decreased Production of RBC's
5. Describe a typical treatment plan for an adult with iron deficiency anemia.
EChing, Tierney, P471
Oral Iron: Ferrous sulfate, 325mg, three times daily, which provides 180mg of iron daily of which up to 10mg is absorbed. Compliance is improved by introducing the medicine more slowly, in a gradually escalating dose with food. Full return to baseline level is expected after 2 months. Should continue for 3-6 months after restoration of normal hematologic values in order to replenish iron stores.
Parenteral iron: The dose (total 1.5-2 g) may be calculated by estimating the decrease in volume of red blood cell mass and then supplying 1mg of iron for each milliliter of volume of red blood cells below normal. One should then add approx. 1g for storage iron. The dose may be given as an IV infusion over 4-6 hours. A test dose of a dilute solution is given first, and the pt should be observed during the entire infusion for anaphylaxis. Because of the possibility of anaphylactic rxs, parenteral iron therapy should be used only in cases of persistent anemia after a reasonable course of oral therapy.
6. Identify the typical causes of folic acid deficiency anemia and vitamin B12 deficiency anemia.
Deb/Current (blue book) pg. 475-477
Causes Folic Acid defic =inadequate dietary intake, alcoholics, anorectic Pts./ people who don’t eat fresh fruits and veggies, and those who Overcook their food are at risk.
Causes of B12 defic= most common assoc. with pernicious anemia/vegans (vegetarians who don’t eat any meat products-including milk, eggs, etc.), abdominal surgery (gastrectomy will eliminate that site of intrinsic factor production)

If you don’t get this within two days, I did not receive your work.
There are any number of reasons why this may happen, so call me at home or talk to me in class and we’ll work it out.
Thanks everyone.
